1. Do you have a fever or have you felt hot or feverish recently? 2. Are you having shortness of breath or other difficulties breathing? 3. Do you have a cough?
4. Any other flu-like symptoms, such as gastrointestinal upset, headache or fatigue?
5. Have you experienced recent loss of taste or smell?
6. Are you in contact with any confirmed COVID-19 positive patients? 8. Have you traveled outside the United States by air or cruise ship in the past 14 days?
9. Do you have heart disease, lung disease, kidney disease, diabetes or any autoimmune disorders?
10. Have you traveled in the past 14 days to any regions affected by COVID-19?

We will be requesting that you check in from your car by text and we will text you when to come in. Please wear a mask when coming to the office. We request that only the patient comes in unless they are very young or the parent/guardian feels they need to accompany their child. If you do accompany your child, you will be required to wear a mask.

3) Evaluators must use a face shields and/or masks while getting the client out of the car - Parents will leave their cell phone number to be called or texted once the evaluations have been completed.
4) Parents will sign the PCEA waiver
5) Clients will bring their own drinks and snacks. Clients will only be allowed to leave the office to use the restroom
6 Encourage clients not to touch their face
7) Encourage clients to keep six feet away from people
8) Once in the office, have the client sanitize their hands
9) Client must wear a mask while seated in the chair and behind the shield
10) Use plastic covers on testing materials and these will be thrown away when done
11) When the client leaves, the following surfaces will be cleaned with paper towel and disinfectant spray
